eRx (Surescripts)
Search 'Village Pharmacy & Compounding' in your EHR's Surescripts directory. Our NCPDP and NPI are listed; if you can't find us, check the Miami / 33176 zip and our team can confirm the IDs by phone.
Fax
Provider fax: 305-200-3783. Include patient DOB, prescriber DEA, and a callback number. Fax orders are reviewed in the same intake queue as eRx.
Secure upload
For practices that prefer a portal upload, our secure upload tools are actively being developed. Open a provider account and we'll set you up with the best current option for high-volume sending.
Call-in
Call 305-274-8955 during business hours and a pharmacist takes the order directly. Best for urgent compounds or when you want to talk through a custom formulation.
